From Addiction to Awakening: A Transformation Case Study
A 5-year case study documenting the trajectory from opioid addiction through ibogaine treatment to sustained recovery and spiritual practice.
The client, a 32-year-old man with a 10-year opioid use disorder (multiple failed treatments, including inpatient rehab, MAT, and therapy), underwent an ibogaine treatment in a medically supervised setting. This case study documents the treatment and the 5-year follow-up period.
The ibogaine session (18 mg/kg) produced a 36-hour experience that the client described as 'a review of my entire life, seen from above.' He reported encountering the childhood trauma that had driven the addiction, experiencing a release of the shame that had maintained it, and emerging with what he called 'a clean slate — not empty, but clear.' Acute withdrawal symptoms were significantly reduced, as is typical with ibogaine.
The 5-year follow-up is the significant part of this case. The client did not return to opioid use, but the sustained recovery was not attributable to the ibogaine alone. In the year following treatment, the client entered a contemplative community, adopted a daily practice, trained as a peer counselor, and rebuilt relationships. At 5 years, he reported: 'Ibogaine opened the door. The last 5 years have been walking through it.' The case illustrates that even dramatic single-session interventions do not produce lasting change in isolation. The ibogaine created a window; the life the client built in the following years is what made the change durable. The medicine is the beginning, not the end.
